ABSTRACT

Intra parenchymal (intra cerebral) haemorrhage is the most common type of spontaneous intracranial haemorrhage. It accounts for 10% of all strokes with a 50% case fatality rate. Hypertension is the one of the most common causes of intra parenchymal haemorrhage followed by other causes like, amyloid angiopathies, cocaine abuse, haematologic disorders and head injury. The present case is a case of death due to intra parenchymal haemorrhage where the relatives of the deceased alleged the death to have been caused as a sequel to an assault by neighbours. Autopsy conducted by experts proved that death was due to pathology and not trauma, thereby assisting in establishment of justice. Usually a single deep seated intracranial haemorrhage in ganglionic region without any injury to scalp, skull and brain is due to natural disease. Hypertension is the most common cause of spontaneous intracranial haemorrhage.

ABSTRACT

Descrevemos o raro caso de um paciente de 45 anos portador de lesão expansiva intracraniana por angiopatia amilóide cerebral com características clínicas e de imagem compatíveis com um glioma de baixo grau. A biópsia revelou angiopatia amilóide cerebral. Os achados clínicos, radiológicos e histopatológicos são discutidos e analisados juntamente à literatura disponível.

ABSTRACT

OBJECTIVE@#To investigate the structural characteristics of the cerebral small vessels with an unknown type of pathological lesion (UTPL).@*METHODS@#Contents of beta-amyloid, alpha-actin and collagen IV in cerebral small vessels with UTPL were studied by Congo red staining, immunohistochemical staining and computer image analysis.@*RESULTS@#The low expression levels of alpha-actin and collagen IV (P<0.05) were observed in tunica media of the vessels with UTPL, and no positive expression of beta-amyloid (P>0.05) was observed in these vessel walls. The expressions of proteins mentioned above in UTPL were different from those of cerebral amyloid angiopathy(CAA) and hyaline arteriolosclerosis.@*CONCLUSION@#UTPL was different from CAA or hyaline arteriolosclerosis in pathologic feature.

ABSTRACT

Hay diferentes enfermedades que cursan con depósito de amiloidosis sistémica. Este material se deposita en los vasos pequeños corticales y de las leptomeninges y generalmente se manifiesta con demencia o hemorragia. Se informa un caso que cursó clínicamente con demencia, y dos que murieron como consecuencia de una hemorragia parenquimatosa extensa. Todos tenían angiopatía amiloide. Se revisan los conceptos sobre angiopatía amiloide, las enfermedades en las que se presenta y el posible mecanismo de la proteína beta, componente del amiloide de la mayor parte de las entidades con amiloidosis cerebral primaria
